Occupational Health onboarding requirements are currently transitioning from ReadySet to Workday and will go live in Workday on 2/23/2024. Newly hired employees should hold vaccine documentation and submit information into Workday beginning Feb. 23.  Occupational Health is not collecting documentation at this time.

Occupational Health Services welcomes you! As part of your onboarding, your immunization records and history will be reviewed in relation to your job duties. 

Complete your Health History Form

Occupational Health onboarding is completed entirely in Workday. All employees with onboarding requirements for Occupational Health will receive a “New Employee Health History” survey in their Workday MyTasks as group 4 of onboarding as seen in the onboarding process steps. The survey will consist of questions related to the relevant heath history and their role and duties at WashU.

If you do not have access to a computer, ask your supervisor or contact Occupational Health to print the paperwork for you to complete.

Complete your Consent for Treatment Form

The Consent for Treatment Survey is required for all workers that may need to use Occupational Health services due to a workplace accident or exposure. This will be assigned to workers as part of onboarding based on Job Profile. This will need to be completed annually. A tutorial for completion of this form on Workday can be found here.

Upload Immunization Requirements

After completion of the Health History Survey, Workday will launch required vaccines in your inbox*. You can find tutorials for uploading evidence of these vaccines here. If you need to receive any of these required vaccines or a titer test to show immunity you can contact occupational health to schedule an appointment at our clinic at the Center for Advanced Medicine. All vaccines/blood tests required for employment will be provided free of charge.

* Some vaccines required for specific functions will be loaded manually by Occupational Health staff after reviewing your health history survey. New employees will be notified of any additional requirements via WorkDay inbox notifications within 10 days of completion of the survey.

Medical Evaluation Questionnaire for Respirator Use

Providers that are required to wear N-95 respiratory protection need to have an initial medical evaluation and approval prior to wearing an N-95. The employee will need to request a medical evaluation through Workday and then complete the OSHA medical evaluation survey, by following the instructions found here.

Complete Your Animal Handler Form

Employees who indicate on their New Employee Health History Form that their duties will involve exposure to research animals will also receive an Animal Handler survey in their Workday inbox. A Workday tutorial for completing this form can be found here. After completion of this form employees may have additional vaccine requirements added to their Workday inbox.

Information about Influenza (mandatory) and Covid-19 Vaccination (encouraged)

Influenza Vaccination

All WashU faculty, staff and students with access to the WUSM campus and other medical facilities are required to receive the annual influenza vaccine with limited exemptions. For additional information see the Influenza Vaccination Policy.

COVID-19 Vaccination

Effective 8/1/2024 new employees are no longer mandated to be vaccinated against COVID. We do strongly recommend that all employees receive annual COVID vaccines, which are available free of charge through Occupational Health. More information can be found here.

Vaccination Requirements for Healthcare Personnel

Those who work in a hospital/clinic or patient care area, or who have direct contact with patients or research study participants.

Measles, Mumps, and Rubella Immunity
  • Written, authentic documentation of two doses of measles, mumps and rubella (MMR) vaccines, at least 28 days apart, after the first year of life
  •  Or laboratory evidence of immunity
  • Employees born before 1957 are required to have titers checked but vaccination is not required, unless under outbreak conditions.
Tuberculosis (TB)
  • Two TB skin tests or one Interferon-Gamma Release Assay (IGRA, the blood test for TB infection) within the past year.
  • If the employee has had a positive test in the past, they must provide proof of the positive TB test and last chest x-ray report.
  • The Bacillus Calmette–Guérin (BCG) vaccine is not a contraindication to testing; IGRA testing is able to differentiate between BCG vaccination and latent TB infection. 
Varicella
  • Written, authentic documentation of two doses of varicella vaccine, at least 28 days apart, after the first year of life
  • Or laboratory evidence of immunity
  • Or diagnosis of varicella disease by a health-care provider,
  • Ordiagnosis of herpes zoster by a health-care provider.
Tetanus, Diphtheria, and Pertussis (Tdap)

Written, authentic documentation of one dose of Tdap Vaccine

Hepatitis B

Staff at risk for occupational exposure to blood, body fluids, contaminated sharps or human cell lines must be vaccinated or decline within 30 days of beginning work. If employees chose to decline, a declination form must be signed.

Color Vision Testing

Employees in a dialysis role will be required to undergo Color Vision Testing during their onboarding period and will see this requirement in their Workday inbox. Instructions for completing the process can be found here. Color Vision Testing must be completed in person at the Occupational Health Clinic at the Center for Advanced Medicine, suite 5A. After an employee completes the initial request process in Workday, Occupational Health staff will contact them to schedule a time to complete the test.

MRI Safety Survey

Employees who will be potentially exposed to high levels of magnetic energy must complete an MRI Safety Survey during onboarding and then annually thereafter. Additional information and instructions for completing this survey in Workday can be found here. If you are required to complete this process you will be notified by your supervisor and/or the Director of Radiation Safety.
