Influenza Vaccination (mandatory)

All WashU faculty, staff and students are required to receive the annual influenza vaccine with limited exemptions. For additional information see the Influenza Vaccination Policy.

You are required to have a current, seasonal influenza vaccination or approved religious or medical exemption, from October 1st to March 31st within 10 business days of employment. Once you have the appropriate documentation, upload to your employee health electronic record in ReadySet.

Upload flu vaccine documentation

  1. Take a picture or scan your influenza proof of vaccination (Formats accepted: pdf, png, jpg).
  2. Log in to your ReadySet account and go to the ‘My Health’ tab.
  3. Choose ‘Documents’ from the side navigation and ‘Upload Document.’
  4. Click ‘Select a File’ to upload the document.
  5. Name the file in the “’Document Name’ field.
  6. Select ‘Document Type’ and ‘Influenza Vaccination.’

COVID-19 Vaccination (mandatory)

All new WashU employees are required to provide proof of full immunization with an FDA-authorized or World Health Organization approved COVID-19 vaccine or an approved religious or medical exemption. For additional information see the COVID-19 Vaccination Policy.

Employees must complete the one-dose vaccine or two-dose vaccine series, or have an approved vaccine exemption within 30 days of employment. Schedule your vaccine with WashU by emailing covidvaccine@wustl.edu. Once you have documentation, upload to your employee health electronic record in ReadySet.

  2. Log in to your ReadySet account and go to the ‘My Health’ tab.
  3. Choose ‘Documents’ from the side navigation and then click ‘Upload Document.’
  4. Click ‘Select a File’ to upload the document.
  5. Name the file in the ‘Document Name’ field.
  6. Select ‘Document Type’ and ‘COVID-19 Vaccination.’

Other Requirements

Separate vaccinations, blood tests and/or medical surveillance may be needed for those working with infectious agents, hazards or animal species in a research setting. Please talk with your supervisor if you are unsure which immunization and surveillance protocol is required for your specific work location.


Please be aware that some services are provided exclusively to WashU Medical School employees, while others exclusively to Danforth Campus employees. Some are provided to both. Please contact Occupational Health if you have questions regarding your status and service needs.
