Requirements by Job Type

After completing the New Employee Health History survey and the Animal Handler survey (if applicable) in Workday each employee will be assigned to one of 12 Occupational Health Programs with the following requirements. The employee will then receive a Workday notification listing these requirements as well as weekly reminders until all requirements are complete.

Clinical Employees
  • Measles, Mumps, Rubella
    • 2 doses of MMR vaccine received after first birthday and at least 28 days apart, or
    • Positive Titer result for each
  • Tetanus, Diptheria, Pertusis (TDAP)
    • received within the past 10 years
  • Varicella (Chickenpox)
    • 2 doses of Varicella Vaccine, or
    • Positive varicella titer confirming prior illness or vaccination
  • Tuberculosis Test completed within past 6 months
    • T-Spot or QuantiFERON-TB Gold blood test, or
    • 2 PPD skin tests completed 14 days apart
  • Annual Influenza Vaccine
Clinical Employees w/Risk of Bloodborne Pathogen Exposure
  • All requirements of Clinical Employees
  • Hepatitis B
    • 2 doses of HEPLISAV-B Hepatitis B vaccine received 28 days apart, or
    • 3 doses of all other Hepatitis B vaccines, or
    • Positive Hepatitis B Titer test
Dialysis
  • All requirements for Clinical Employees w/ Risk of Bloodborne Pathogen Exposure
  • Color Vision Testing
Animal Handler – Rodents, Fish, and Amphibians
  • Annual Influenza Vaccine
  • Tetanus, Diphtheria, Pertussis (TDAP) vaccine received within past 10 years
Animal Handler-Large Breeds
  • Annual Influenza Vaccine
  • Tetanus, Diphtheria, Pertussis (TDAP) vaccine received within past 10 years
  • Annual Q-Fever Titer
Animal Handler – Non-Human Primates
  • Annual Influenza Vaccine
  • Tetanus, Diphtheria, Pertussis (TDAP) vaccine received within past 10 years
  • Herpes B Card (provided by Occupational Health)
  • Rubella Titer
  • Rubeola (measles) Titer
  • TB Test, annually
    • T-Spot blood test provided by Occupational Health
Animal Handler – Veterinarian/Vet Tech
  • All Requirements for Animal Handler – Non-Human Primates
  • Q-Fever Titer, annually
  • Rabies Vaccination + Rabies Titer completed every 2 years
Laboratory Workers w/exposure to Human Cell Lines
  • Annual Influenza Vaccine
  • Hepatitis B
    • 2 doses of HEPLISAV-B Hepatitis B vaccine received 28 days apart, or
    • 3 doses of all other Hepatitis B vaccines, or
    • Positive Hepatitis B Titer test
Dietary
  • All Requirements for Clinical Employees
  • Hepatitis A
    • 2 doses of Hepatitis A vaccine received 6 months apart
Housekeeping
  • Annual Influenza Vaccine
  • Hepatitis B
    • 2 doses of HEPLISAV-B Hepatitis B vaccine received 28 days apart, or
    • 3 doses of all other Hepatitis B vaccines, or
    • Positive Hepatitis B Titer test

Security
